For your safety and to comply with insurance regulations, various forms MUST be submitted before you can attend many of our activities.  Filling out forms is never fun BUT at least they are easy to find!  You've come to the right page.  Click on the form you need to download a pdf file which can be viewed or printed from your desktop.

Permission Slips are mandatory for every activity except the regular meetings.

The Visitor Slip is for when you invite a friend.  We just need to know who is with us.

The Health Form is required for the entire program year and for any trips we take.

Every parent should have a Transportation Form on file to help with rides.
